SDSU Shipping Pickup Request Form

Contract and Procurement Management at 4-5243 must be notified first if materials requested to be shipped were obtained via State PO's for service, exchange, or return. All boxes to be shipped must have a shipping request form attached (each).

Just complete the form below to arrange your pickup. Provide us with as much information as possible. Be sure to click on the "Pickup" button to submit your request. Your computer will need to have e-mail to use this online form. Our staff should arrive on or before the next business day to pickup your shipment.
